Introduction

As early as the middle of the 19th century, the German pathologist Rudolf Virchow discovered a large infiltration of inflammatory cells in tumour tissue and proposed the conjecture that tumours originated from chronic inflammation. It is now clear to the medical community that inflammation is closely related to cancer development, progression and the efficacy of anti-cancer treatment. For example, gastric cancer is associated with H. pylori infection, nasopharyngeal cancer with herpes virus infection, and liver cancer with hepatitis virus infection; systemic inflammation, such as obesity and depression, is also associated with tumour incidence and poor efficacy of anti-cancer treatment; and treatment-induced inflammatory responses also affect the clinical outcome of tumours to varying degrees. Although some anti-inflammatory drugs (aspirin, statins) can significantly reduce the incidence of cancer and the risk of death, some pro-inflammatory cytokines or stimulants (TNF-α, cGAS-STING pathway activators) can promote the infiltration of immune cells into infected tissues and thus significantly improve the efficacy of tumour treatment, suggesting that inflammation is a "double-edged sword This suggests that inflammation is a "double-edged sword".

Inflammatory processes are finely tuned systems, with many molecular signals that orchestrate the immune response. For most conditions, it is probable that only limited diagnostic information will be obtained by one or two blood-derived inflammatory markers. The potential strength of molecular imaging is to 'see' into the core of the inflammatory site, targeting multiple biomarkers (multiplexing) to provide a deeper insight into the triggers, cellular interplay, pro- versus anti-inflammatory mediators and the ability to evaluate current and the next generation of therapies for chronic immune diseases with a level of precision and detail not seen before. Implementing molecular imaging, therefore, can uncover new information on disease processes which can in turn offer new targets for disease diagnosis and stratification.

After intravenous injection, 67Ga is bound to transferrin and transported to the site of inflammation to localise the aggregates in the inflammatory lesion.

Adherence to the vascular endothelium due to a mechanism of increased expression of local adhesion molecules in inflammation. Subsequently, they migrate through the endothelium and basement membrane by a process of cellular exudation and by chemotaxis to the inflammatory lesion. The site of the inflammatory lesion can be revealed by in vitro detection of the radioactive distribution.
